There's a missing bit of concrete next to the manhole cover, causing a raised edge.
There is also a thin strip of paving slab missing or sunken nearby (see arrows) that could also have caused the trip.
I saw the lady fall from a distance so not sure which one caused this. She fell onto both knees and was clearly in some pain as a result.

Potholes on Wells Road, 29th April
When are the several potholes in the Wells Road going to be filled in? They are dangerous to cyclists and can wreak a car’s suspension. Traveling up this road in a bus is a nightmare. Every pothole makes the wheels drop about 3 inches and is extremely uncomfortable for passengers. Also the bus feels like it’s falling apart!

Signpost very loose, 29th April
The kids from the school have nearly pulled out the signpost beside the gate to Lyncombe Fields. It's very wobbly and feels like it's about to fall down. The pavement around it has risen up.

Lots of weeds on road, pavement and property walls - Entry Hill Park
Weeds going up pavements and side of road and it's very unsightly. Please could this be cleared as it's causing issues for the road surface. This is very untidy whereas other streets in Bath are very clean and tidy.

Paving slab on Great Pulteney Street badly broken, 29th April
Paving slab outside number 15 is badly raised and caused a resident to trip and hit a lamppost, and had to go to A and E for her injuries/ concussion. Please can this be fixed asap?

Several broken paving slabs on public access, 28th April
There are several broken paving slabs with some concaving inwards creating a 5cm tripping hazard. This pavement is public access to my privately owned property which very soon will have many pedestrians walking up and down when viewing my property to rent, including small children. My estate agents have informed me that this is council owned and indeed extremely hazardous.

Blocked drain causing flooding on pavement and private driveway, 27th April
This is an ongoing issue that does not get fixed. The problem is getting worse as the drain is not maintained as well as failing at dealing with surface water. It now floods onto our property which is not acceptable.

The bin is full to overflowing and there was a bag of rubbish strewn over the cycle lane and footpath,. That has been cleared by a shopkeeper, 27th April

Smashed bottle right on path near crossing, 26th April
Right by the crossing button , the crossing by the hotel . This is used every school day by infant and junior children plus toddler siblings . Yesterday a child got glass stuck in their trainer . Please can you clean up . Thank you
